Immunization Waivers for 2018-2019

Just a reminder that as students register in the fall, waiver forms can no longer be filled out at school. All waivers must be
completed at the health department.
 
Parents need to call the Immunization Clinic at the Monroe County Health Department (734) 240-7800 to make an appointment to
sign a waiver form. They should do this as soon as possible, as the appointments will be filling just before school starts.
 
Due to a NEW STATE LAW - ONLY waivers completed through the health department and signed by health department staff will
be accepted.
